Authorization and consent

DMV Driving Abstract. I authorize Astra to obtain my complete NY State driving abstract from the Department of Motor Vehicles for the purpose of evaluating my qualifications under Article 19-A of the Vehicle and Traffic Law.

Statewide Central Register Check. I authorize Astra to submit a Statewide Central Register (SCR) database check to the New York State Office of Children and Family Services to determine whether I am the subject of an indicated report of child abuse or maltreatment.

Truthfulness. I affirm that all information provided on this application is true and complete to the best of my knowledge. I understand that knowingly providing false information may be grounds for denial of employment or dismissal.

Next steps. If my application is accepted, I understand I will be asked to complete post-offer onboarding, which includes additional identity verification, a physical examination, fingerprinting if required, and federal employment forms (I-9, W-4).
